Entering Scone was interesting. I hadn't looked at the data on my medical arm band properly for 7 years. However, EFA say to purchase a new one to have a nice clean page to write on. Nothing wrong with my card holder - it is leather. So, I typed up a new form to put in the holder. Being a good citizen, and with the interest of your health and safety in mind, I decided to put it on this site for all to download and update easily. However, I cannot work out how to attach a file. How do I do this? Ah, perhaps no-one would want it anyway.......

Just go to the Eventing NSW website and down load it from the forms section.

It is there under forms....maybe you need to refresh the site, especially if you have it saved to your 'favorites'.
The form is in 'word' so you can type the answers in and save it for future FEI events.
If you want you can also send a copy to me and I can return it to the Entry Secretaries when I return the entry checks for each FEI event.

Ah - I see thanks so much Vicki. I should have asked in the first place before typing it up! (Well I did ask the EFA, wrong choice).
One question - do you think we should be putting health fund information on there (membership, health fund provider and phone number)? Perhaps Vince could comment?

I will ask Vince to comment, but I think it would be best if you gave your health insurance details to the 2 contacts you have provided on the form.
If you were unconscious the doctor/paramedics would contact your contacts, but you would receive emergency medical treatment irrespective of health insurance status. If not unconscious you could provide the hospital with details.

Yes, it can be nice to decide if you actually want to be a private patient or not...as you can often get the best emergency care in the public system.

I thought at one point we were supposed to at least have ambulance cover?? I have full cover so I dont mind but I swear I read that somewhere possibly at a Berrima Event!!

If you don't have ambulance cover OR are not in a private health fund, you wil;l be charged $800-1000 for a quick ride to the hospital and $10,000 for a helicopter ride. pays to belong! Vince
